StayWell Health is committed to providing quality health care and service to all patients. In order to continue this mission, it is essential that payment be received at the time of service. We will submit claims to any insurance company according to established billing guidelines. To do this in an efficient manner it is important that patients provide their insurance information at the time of service. We accept Medicare, Medicaid, Husky and Private insurance. Patient and/or the responsible party will receive a monthly statement which explains any activity occurring since the last service.
To assist patients in meeting their financial obligations, StayWell Health provides the following programs:
We will honor Debit, Visa, MasterCard for payment of services. These payments will be accepted either by person, by phone or by mail. Personal checks and money orders are also accepted.
SLIDING FEE DISCOUNT PROGRAM (SFDP)
StayWell Heath Center provides health care to all individuals, regardless of ability to pay. We offer a Sliding Fee Discount program for all patients with or without insurance at or below 200% of federal poverty guidelines. You may apply for the discount program at time of registration. The amount of the sliding fee discount is based on your family household size, household income, and determined using current Federal Poverty Level income guidelines. Please inquire about program requirements and application assistance with our bilingual registration staff. Interpretation services available. You will be asked to provide proof of income (W-2’s, Individual Tax Return 1040, copy of unemployment checks, last 2 pay-stubs or letter from your employer). The proof you supply will be used to calculate your discount.
StayWell Health Center has an Outreach Department that will assist you in applying for third party assistance such as: Medicaid and Husky A, B, C and D. Please ask our Front Office Receptionist for information.
PAYMENT PLAN ARRANGEMENTS
