How to Give

Charitable gifts of any amount go a long way toward helping a community health center give top quality care and treatment to its large patient population. Since StayWell Health Care, Inc. is a not-for-profit organization, your gift will be tax-deductible.

Capital Campaign

Thanks to the generous support of our community partners, StayWell was able to move into our new building and purchase equipment and furniture. We thank the following donors:

Through the Opening New Doors to Community Health Campaign, the StayWell Health Center of Waterbury, Connecticut embarks on an exciting new chapter in its history.

After many years of searching for a location that would combine the two North End locations of 232 North Elm Street and 80 Phoenix Avenue, StayWell Health Center has found a building: 80 Phoenix Avenue. StayWell currently occupies a suite located on the first floor of the Phoenix Professional Plaza building offering primary medical for adults and pediatrics.

StayWell has experienced rapid growth in its client base. In 2000, StayWell provided 14,856 health care visits. Five years later in fiscal year 2005, we provided 50,557 visits, a growth of 340%. StayWell employs 108 health professionals to care for these patients.

In August 2006, the “Opening New Doors to Health Care” Capital Campaign was launched. This capital campaign plan is designed to support the purchase of The Phoenix Professional Plaza (80 Phoenix Avenue) located in the North End of Waterbury. This site will combine 2 of our existing sites in an integrated way that creates efficiency in operation costs, improved service delivery and expanded access. Our strategic plan requires that we be in a strong financial position and prepared to expand services as the uninsured are engaged into care. A large mortgage would be burdensome and limit our ability to expand without overwhelming risk. We are seeking community partners to contribute to this worthwhile campaign.

Through the “Opening New Doors to Health Care” campaign, there are opportunities for sponsorship of an entire suite (i.e. Pediatric Suite), a room within a suite (an examination room in the Women’s Health suite), equipment (chairs for the Adult Medicine waiting room), etc. Sponsors will have naming privileges based on level of sponsorship.

Memorial and Tribute Gifts

Memorial and tribute contributions to a local community health center provide a special opportunity to let family and friends know they are being remembered.

A memorial gift is a thoughtful and caring way to express sympathy while sharing concern for the health and well being of others.

A tribute gift to a community health center provides the unique opportunity to honor special people on birthdays, graduations, weddings and other memorable occasions. It is a great way to show a family member or friend that you share and respect their love for the health and happiness of people from all walks of life.

Other Ways to Give

Planned Gift
A planned gift to StayWell Health Care, Inc. may be arranged by setting up a bequest, charitable gift annuity, endowed gift or another planned giving vehicle. Talk with a lawyer for more information.

Donation for services or supplies
Have an impact on health care or the quality of life for someone in your community. You can make a monetary donation to be used for:

the cost of a dental visit for an uninsured child or adult
the cost of a prenatal visit for an uninsured family
the cost of a family planning visit for an uninsured individual
the cost of a test for chronic illness such as diabetes, hypertension, etc.
the cost of prenatal lab work and/or ultrasound for family in need
the cost of an annual women’s health visit
purchasing supplies for a newborn baby for a patient in need
buying children’s books for the Community Programs’ lending library or Pediatric waiting rooms
purchasing toys or clothes during Winter holidays for a patient in need

Donation of goods
StayWell Health Care, Inc. also accepts donations of r the following items:

Gently used magazines or books for children or adults
Unopened package of stickers
Unused office supplies
Unopened diapers and baby wipes
Gently used or new toys
Gently used or new items for newborns
Unused crayons, magic markers and coloring books
