Charitable
Gifts
How to Give
Charitable gifts of any amount go a long way toward helping a
community health center give top quality care and treatment to
its large patient population. Since StayWell Health Care, Inc.
is a not-for-profit organization, your gift will be tax-deductible.
Capital Campaign
Thanks to the generous support of our community partners, StayWell was able to move into our new building and purchase equipment and furniture. We thank the following donors:
American Savings Foundation
Connecticut Community Foundation
Naugatuck Saving Bank Foundation
The Leever Foundation
Watertown Foundation, Inc.
In August 2006, the “Opening New Doors to Health Care” Capital
Campaign was launched. This capital campaign plan was designed
to support the purchase of The Phoenix Professional Plaza (80
Phoenix Avenue) located in the North End of Waterbury. This site
combined the 2 sites located in the North End of Waterbury in
an integrated way that created efficiency in operation costs,
improved service delivery and expanded access. In November 2004,
StayWell completed the move from their former site located at
232 North Elm Street and successfully settled into their new
location located at 80 Phoenix Avenue.
Sponsorship opportunities through the “Opening New Doors
to Health Care” campaign are still available. Sponsors
will have naming privileges based on level of sponsorship such
as sponsorship of an entire suite (i.e. Pediatric Suite), a room
within a suite (an examination room in the Women’s Health
suite), equipment (chairs for the Adult Medicine waiting room),
etc.

Memorial and Tribute Gifts
A memorial and tribute contribution
to a local community health center provides a special opportunity
to let family and friends know they are being remembered.
A memorial gift is a thoughtful and caring way to express sympathy
while sharing concern for the health and well being of others.
In 2006, the Benjamin “Buddy” Fabian Memorial Fund
was established to honor the memory of this beloved StayWell
employee by his family, co-workers and friends. At the request
of the Fabian family, this memorial fund was merged into StayWell’s “Opening
New Doors to Healthcare’ capital campaign.
In 2007, family and friends honored the memory of Louise Fasono
by establishing a memorial fund. Funds from the Louise
Fasono Memorial Fund were used to establish a nutritional program for
the students of Driggs Elementary School to honor the former
cafeteria worker.
A tribute gift to a community health center provides the unique
opportunity to honor special people on birthdays, graduations,
weddings and other memorable occasions. It is a great way to
show a family member or friend that you share and respect their
love for the health and happiness of all individuals within the
community.
Gifts can be targeted in specific ways such as:
patient fund,
operating cost,
staff training,
medical supplies,
parenting programs,
new equipment,
a specific area, such as asthma, diabetes care or dental
services,
a specific population, such as children, pregnant women
or adults.
